Population ageing has diverse aspects and multiple consequences. With the transition to “aging from the top” the importance of medical aspects of aging, health issues and active longevity of elderly people increases. The article evaluates the older population’s health status in the Republic of Komi in the new campaign of medical examination of adult population in the context of the purposes stated in Russia in the field of life expectancy. Sources are the official statistics, the annual “Government reports on the health status of the population of the Komi Republic” and the results of the two sociological research “Problems of the third age” conducted in 2013 and 2018. It is established that in the Komi Republic and in Russia as a whole there are still considerable reserves for extending the positive trend of increasing life expectancy: by causes of death, gender, rural, regional etc. In the context of accelerating the population’s aging the increasingly significant role is played by mortality reduction at older ages, defined by the preservation of health, which is inextricably linked to health care development including specialized medicine, with the strengthening and improvement of healthy lifestyle promotion, formation of responsible attitude of citizens of all ages to their health, prevention of major modifiable risk factors of chronic disease with early detection and adequate treatment, with prolonging the duration of the population’s full, active life. The study revealed the positive impact of population’s medical examination on the older people’s awareness about their health increasing their responsibility for it, strengthening the independent component of orientation to an active, healthy lifestyle, increasing motor activity. At the same time, at older age there is an increase of problems associated with diabetes, unbalanced and excessive nutrition, overweight and obesity
